Create Risk Matrix
Your Risk Matrix
Add risks above to plot them on the matrix
How to Use This Tool
Create a risk assessment matrix in three simple steps:
- Choose matrix size — Select 3×3, 4×4, or 5×5 depending on how granular your assessment needs to be.
- Add risks — Enter a risk name, select its likelihood (probability of occurrence) and impact (severity of consequences), then click "Add Risk."
- Review and export — See your risks plotted on the matrix with color-coded severity. Download as PNG or SVG for reports and presentations.
Understanding the Risk Matrix
A risk matrix (also called a probability-impact matrix) is a visual tool for prioritizing risks based on two dimensions:
- Likelihood (X-axis) — How probable is it that the risk will occur? Ranges from Rare to Almost Certain.
- Impact (Y-axis) — If the risk occurs, how severe are the consequences? Ranges from Negligible to Severe.
The risk score is calculated as Likelihood × Impact. Higher scores indicate risks that need more urgent attention.
Risk Severity Levels
| Level | Score Range (5×5) | Action |
|---|---|---|
| Low | 1–4 | Accept or monitor |
| Medium | 5–9 | Mitigate if practical |
| High | 10–16 | Action required |
| Critical | 17–25 | Immediate action required |
When to Use a Risk Matrix
- Project management — Identify and prioritize project risks during planning
- Business continuity — Assess threats to operations
- Safety assessments — Evaluate workplace hazards
- IT security — Prioritize cybersecurity threats
- Strategic planning — Evaluate business risks and opportunities
Tips for Effective Risk Assessment
- Involve multiple stakeholders for more accurate assessments
- Be consistent in how you define likelihood and impact levels
- Document the reasoning behind each risk's placement
- Review and update the matrix regularly as circumstances change
- Focus attention on the top-right quadrant (high likelihood + high impact)
- Don't ignore low-probability, high-impact risks — they can be catastrophic
Frequently Asked Questions
What matrix size should I use?
A 5×5 matrix provides the most granularity and is the industry standard for detailed risk assessments. Use 3×3 for quick, high-level assessments or when you have fewer risks to evaluate.
How do I determine likelihood and impact?
Base your assessment on historical data when available, expert judgment, and industry benchmarks. Be specific about what each level means for your organization. For example, "Likely" might mean "expected to occur once per year" in your context.
Can multiple risks share the same cell?
Yes! The tool automatically offsets overlapping risks so you can see all of them. Each risk is numbered and listed below the matrix for reference.
What's the difference between inherent and residual risk?
Inherent risk is the risk level before any controls or mitigations. Residual risk is what remains after applying controls. Consider creating two matrices — one for each — to show the effectiveness of your risk management.
